29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about May 21, 2014 at worksite 1177 N. Morley in Moberly, Missouri- employees were exposed to a struck by hazard from a Dayton Industrial Grinder, in the welding room, which had tongue guards that were improperly adjusted in excess of one-fourth inch. Abatement certification is not required for this item:

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective: On or about May 21, 2014 at the Orscheln Products facility, employees were exposed to Electrical Shock injuries, in that the electrical extension cord on the industrial fan in use did not have a ground pin to ensure the path to ground was permanent, continuous and effective. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about May 21, 2014 at the Orscheln Products facility- a 220V flexible cable had pulled loose from its attachment point to a portable outlet box leaving the cable attached to the device by its internal conductors.
