29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:         a)  Metal section at the Glass Shop:  On or about May 22, 2014 employees were exposed to flying chips and rotating parts while they were drilling holes in aluminum stock on a JET milling machine, SN 8127102, model CS-18 and the rotating chuck was not guarded.         b)  Metal Shop:  On or about May 22, 2014, employees were exposed to flying chips and spark while they were boring holes in aluminum stock to create window or door frames on a Yeong Chin Machinery Mill machine, SN 087674, model YC-1AVS  and the rotating chuck was not guarded.         c)  Glass Shop, rear of building:  On or about May 22, 2014, employees were exposed to flying chips and rotating parts while they were drilling holes in plastic on a Clausing milling machine, SN 118903, model 1630 and the rotating chuck was not guarded.
