29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:    At this jobsite, employee(s) unloaded spools of wire onto a fourth floor window from a job-made box which sat on top of the forks of a Skytrak Lift Truck.  The box was unsecured and exposed employees to fall hazards when the box was elevated to a height greater than thirty feet above ground.
