OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards where combustible metal dust was generated:    A.  De-burring Room:  On, or about, May 19, 2014, an Easy Bench down draft work bench caught on fire while an employee was de-burring titanium aircraft parts.  The two Airflow Systems, Inc. down draft workbenches, Model Easy Bench ALUM-BF1-MAT, had not been designed or equipped for use with titanium and lacked adequate controls to protect employees from fires and/or explosion hazards that could result from the improper use of the work benches.      B. De-burring Room:  On, or about May 23, 2014, a dust collection system that consisted of ductwork and a Donaldson Torit Model 24 cyclone dust collector that was used to collect titanium, Inconel, stainless steel and abrasive blasting dusts, had not been designed, installed, or maintained to protect employees against fire and explosion hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Plant:  On, or about, May 27, 2014, the following machining centers were not provided with adequate safeguarding:     A.  Milling Department:  The following CNC machines were found to have access doors that could be opened during the automatic mode when the tooling was in operation:    1.  Machine #31, Hyandai-Kia horizontal milling, Model KH636:   The access door interlock was bypassed, the key had been removed from the door and inserted into the receiver.      2.  Machine #4 Fadal, Model VMC4020 milling center:  Half of the access door interlock mechanism was missing and had been removed from the machine.        3.  Machine #1 Fadal, Model VMC4020 milling center:  The access door was not provided with an interlock.      4.  Machine #2 Fadal, Model VMC4020 milling center:  The access door was not provided with an interlock.      5.  Machine #21 Fadal:  The access door interlock had been bypassed.       6.  Machine #18 Fadal:  The access door had a binder clip secured to the interlock to bypass it.        7.  Machine #5 Fadal:.  The access door interlock was bypassed and secured with duct tape.        8.  Machine #12 Fadal:   The access door interlock was bypassed and secured with duct tape.       9.  Machine #17 Fadal VMC 4020:  The access doors to the machining area of the machine were not provided with interlocks and could be opened during the automatic mode.      10.  Machine #30 Fadal VMC 4020:  The access doors to the machining area of the machine were not provided with interlocks and could be opened during the automatic mode.      11.  Machine #14, #13, #16 and #15, identical Compumill HMC-410 milling centers:  None of the access doors were provided with interlocks.      B.  Turning Department:  On, or about, May 27, 2014, the window provided in the access door of Machine #27, a Clausing/Colchester turning center, was shattered due to tool breakage.  The window had not been replaced after the incident and it was unlikely that the window could withstand another similar impact and remain intact.

Easy Bench Model ALUM-BF1-MAT down draft table, serial number 1027948:   The fan unit and its electrical wiring, including a switch connection, had titanium dust coated onto the electrical conductors and switch, all of which were located in the exhaust air stream.  A relocatable power tap was draped over the back shield of the down draft table.       B.  Easy Bench Model ALUM-BF1-MAT down draft table, serial number 1027947:  The fan unit and its electrical wiring, including a switch connection, had titanium dust coated onto the electrical conductors and switch, all of which were located in the exhaust air stream.      C.  Two Craftsman 6 inch pedestal grinders:  Titanium dust had migrated into the on/off switch enclosures.    D.  Wall mounted receptacles:  Two standard wall mount electrical receptacles that were used to energize the two Craftsman and one Baldor pedestal grinding/buffing machines were found to contain dust from the grinding/buffing operation inside of the enclosures.
