29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. This violation occurred on or about May 24, 2014, and at times prior thereto, when employees were exposed to falls from a height approximately 12 feet above a concrete surface. a. On the south-east corner of the building, an employee was observed walking and bending down on a truss approximately 12 feet above a concrete surface without the use of guardrails or fall protection. b. An employee working on an inner wall was observed sitting on a truss 12 feet above a concrete surface without the use of guardrails or fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: This violation occurred on or about May 23, 2014, and at times prior thereto, when employees were using an aluminum extension ladder to access a truss approximate 12 feet off of the ground without the ladder side rails extending at least 3 feet above the upper landing surface being accessed.
