Section 5 (a) (1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees operating aerial lifts: On or about May 22, 2014, and at times prior there to, the employer did not ensure that aerial lifts were properly loaded while operating the aerial lift. The employee was exposed to fall, struck-by, caught-in, and or crushed-by hazards.
