29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by the point of operation:   a)The guard on the Accupress shear was not adjusted to prevent an employee from placing his hand in an area where the compression spring holdown clamp caused a finger injury on April 16, 2014.   b) The pedestal controls on the Betenbender press brake were not secured. A employee could operate the press brake while positioned in an area where they could place their hand into the point of operation causing a serious injury.
