29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a) Drive shaft connected to the Maxi Cure conveyer belt was not guarded to prevent employees exposure to rotating parts.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.      a) The knockouts in the electrical disconnect box for the over head lights was not enclosed to prevent from contact.    b) The knockouts in the electrical disconnect box for the silk screen machines were not enclosed to prevent from contact.    c) The knockout in the top of the electrical receptacle box for the Challenger silk screen machine was not enclosed to prevent from contact.    d) The faceplate for the electrical receptacle box for the Challenger silk screen machine was not covered to prevent from contact.

29 CFR 1910.305(g)(1)(iv)(B): Where run through holes in walls, ceilings, or floors;    a) The cord for the sprayer gun was exposed to damage and/or cuts, in that the cord was pulled through a metal door knob hole.    b) The extension cord for the front office was exposed to damage and/or cuts,  in that the cord was pulled through the metal door frame.    c) The cords in the Silk screen department were exposed to being damage from foot traffic in that the cords were positioned under pieces cardboard.
