29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  Jobsite: Scaffold.  Employees were exposed to falling up to approximately 20 feet onto the sidewalk below while climbing the walk-thru frames of a tubular welded frame scaffold where proper access had not been provided.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  Jobsite: Scaffold.  A competent person had not conducted an inspection and caused abatement where the tubular welded frame scaffold contained several defects: 1) There was a plank missing from the top tier work platform. 2) Proper access/egress had not been provided 3) The scaffold was built around a street light (electrical) cable and neither the state of energization nor the voltage was known 4) There was a worker on the scaffold without a hard hat 5) There were no end rails on the working platform 6) The guard rails on the cross-brace side were incomplete 7) A worker was accepting material at the well wheel without fall protection

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  Jobsite: Tubular welded frame scaffold.  Employees were exposed to falls of approximately 20 feet to the ground below while working from a scaffold where there was:  a) an incomplete guardrail system installed on the cross-brace side b) the end rails were missing on the working platform c) no fall protection was utilized during hoisting wheel operation

29 CFR 1926.451(h)(1): Each employee on the scaffold was not wearing head protection:  Jobsite: Tubular welded frame scaffold.  An employee was exposed to head injury while modifying the scaffold and while masons were working overhead where head protection was not utilized.
