29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: a. On the back side of the building (East side), the employer did not ensure that the temporary lighting was free from recognized hazards, in that the first light fixture was hanging down near the floor level, and the base of the bulb was damaged exposing live parts.

Outside on the South side of the building, an employee was using an 8 foot Louisville stepladder in a manner which it was not designed or intended to be used by leaning the ladder in the closed position against the side of the building.
