While the cover is not in place, the manhole opening shall be constantly attended by someone or shall be protected by removable standard railings: a) Dillsbury Court, Orlando, FL - No rails were used around the open manhole, on or about May 29, 2014. Envirowaste Services Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(a)(6), which was contained in OSHA inspection number 313107922, citation number 1, item number 1 and was affirmed as a final order on April 1, 2010, with respect to a workplace located at 1900 Liberty Avenue, Miami Beach, FL.
