29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Facility Wide-  The employer failed to establish a Lockout Tagout Program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on machines such as the Mazak Turning Machines or other equipment where the unexpected energization, startup or release of stored energy could occur and cause injury.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Assembly Area- The employer failed to ensure that powered industrial truck operators were competent to operate powered industrial trucks, such as the Toyota Forklift, Serial #19075, safely as demonstrated by the successful completion of the training and evaluation.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Machine Shop- The employer failed to ensure that electrical equipment, such as the multiple outlet gang boxes that were hanging from flexible conduit near Mazak 6200Y, Mazak VCS, Mazak Turn 20, and Mazak 6200, was being used as permitted under its listing and labeling.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
