29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) The employer had not developed an energy control program.  An effective energy control program includes, but is not limited to:     1) A written program establishing and describing the employer's energy control program.     2) A general energy control procedure for single energy source equipment where the energy source is          readily identifiable and cord & plug equipment.     3) Specific energy control procedures for multiply energy source equipment and single energy source equipment where         the energy source is not readily identifiable.     4) Affected and Authorized employee training and the associated documentation.     5) Usage of uniquely identified equipment for energy control.     6) Periodic audits of the energy control program.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operators and other employees from hazards created by moving parts:  a) The interlocks for guard doors 1 and 2 on the Automated Lock Builder were engaged, but disconnected from the guard doors. Guard doors 1 and 2 could be opened during operation without shutting down the Automated Lock Builder which allowed employee access to the moving robotic arm and other mechanical hazards. Employees were entering the robotic arm area during production to unjam tumblers stuck in their track.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a) Employees had not received training on the new global harmonized system labeling elements and safety data sheet format.
