29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about May 28, 2014, at the above addressed worksite, the employer did not provide training for preventing employee's exposure to a fall hazard while working on a flat roof without adequate fall protection, as evidenced by among other conditions; employees lacked knowledge of the proper set up of warning line and personal fall arrest systems and other fall protection standard requirements.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems: On or about May 28, 2014, at the above addressed worksite, employees were working on a flat roof without fall protection. White Seal, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 313105264, citation number 1, item number 1 and was affirmed as a final order on December 3, 2009, with respect to a workplace located at 2008 Coolidge St, Hollywood, FL 33020.
