Throughout the facility - On or about May 29, 2014, employees were required to operate powered industrial vehicles during shipping / receiving, and material handling operations. The employer did not ensure that each powered industrial vehicle operator completed the required training and evaluation specified in paragraph (l); thereby by exposing employees to struck-by, caught-in, and crushing hazards.
