29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Connection North East to East Scaffold: Employee was working from a welded frame scaffold platform that was not fully planked, being exposed to a fall of approximately 12 feet, on or about 5/28/13.  b) East Scaffold: Employee was working from a welded frame scaffold platform that was not fully planked, being exposed to a fall of approximately 6 feet, on or about 5/28/13.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) North East Scaffold and East Scaffold: Employees without any safe means of access were working from a welded frame scaffold platform that was approximately 12 feet above the ground, on or about 5/28/14.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a) South Scaffold: Employee without any means of fall protection was working from a welded frame scaffold platform being exposed to a fall of approximately 12 feet, on or about 5/28/14.  b) Connection between North East Scaffold and East Scaffold: Employee without any means of fall protection was working from a welded frame scaffold platform being exposed to a fall of approximately 12 feet, on or about 5/28/14.   c) East Scaffold: Employee without any means of fall protection was working from a welded frame scaffold platform being exposed to a fall of approximately 12 feet, on or about 5/28/14.
