29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. On May 27, 2014, and times prior to, employees were not using proper personal protective equipment in the following areas: a. Employees were working near an acid bath stations were without aprons and/or proper chemical gloves. b. Employees were working near the Zinc kettle were without fire resistant clothing. These conditions exposed employees to chemical and thermal burns.

The employer had not assessed the work place to determine if hazards were present which would require the use of personal protective equipment.
