On or about May 28th 2014, at the above addressed jobsite the employer did not provide the information contained in the appendix D to employees using a 3M 8211 N95 filtering face-piece respirator in the fabrication and painting areas.

On or about May 28th 2014, at the above addressed jobsite, the employer did not provide the appropriate gloves (Nitrile) to the employees who were wearing latex examination gloves when performing Methyl Methacrylate operations.
