29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) Production Workshop Area - on or about May 28, 2014, employees were activating a light switch that was located directly below an unguarded junction box which was its missing cover and was not properly guarded to prevent contact with the exposed live parts energized in excess of 50 volts AC.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Production Workshop Area On or about May 28, 2014, an employee performed various duties in the production workshop area where lockout/tag out hardware is used, but at least an annual inspection of the energy control procedures are not conducted, exposing employees to injury.
