29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: (a) In the Area in and around the Okuma 7269 work center coolant leaks continuously within the work center area and outside onto the main floor creating a slip and fall hazard. (b) In the aluminum plant the FP001 machine, (not operational) was leaking oil onto the main floor aisle creating a slip and fall hazard.

29 CFR 1910.305(b)(1): Openings through which conductors enter boxes, cabinets, or fittings were not effectively closed: (a) On the interior wall in the aluminum die cast building a conduit elbow entering the 09800 Shotscope control box was missing a cover (b) On the interior wall in the aluminum die cast building the conduit entering the duplex outlet supplying power to the employee water cooler was disconnected. (c) On the outside wall to the right of the roll up exit door the cover to the 4 inch outlet box was loose and hanging open.
