29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a)  172 Hilltop Drive Job Site - on or about 5-29-14 - Employee was cutting a doorway in the concrete foundation with a demo saw wearing only sun glasses.  The sun glasses in use were not safety glasses meeting ANSI Z-87.1 standards.

29 CFR 1926.451(b)(2)(ii): Where scaffolds must be used in areas that the employer can demonstrate are so narrow that platforms and walkways cannot be at least 18 inches (46 cm) wide, such platforms and walkways were not as wide as feasible:  a)  172 Hilltop Drive Job Site - On or about 5-29-14 - Employee was using a single planked walkway from the wooden deck to the seven foot high foundation wall that was only 10 inches wide and not provided with a guardrail system exposing employee to fall hazards.
