OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to caught in between hazards:     a)  Municipal Recycling facility:  On or about 05/29/2014 Freehold Cartage Inc. employees were exposed to hazards and injuries created by the failure to properly support the steel door of a 4500 gallon roll-off vacuum tank container while cleaning residual non-hazardous from the interior.  The door of the container was opened using a chain attached to the grapple/bucket of a Caterpillar skid steer.  The chain slipped from the skid steer grapple/bucket, causing the door to unexpectedly close on an employee, killing him.

Observed on or about 6/10/2014.         b) Fabrication Shop:  Employees were exposed to amputation and crushing hazards due to a Cincinnati power press brake with an unguarded opening at the point of operation of approximately one (1) inch.
