29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: a) for the establishment located at 9211 N. Florida Avenue in Tampa (FL) doing business as BravoTampa LLC (Burger King) as noted on 05/12/14. A worker experienced a fatal heart attack while leaving the restaurant after finishing his work shift. The incident occurred on 05/10/14 but was not reported to OSHA until 05/12/14.
