29 CFR 1910.119(m)(1): The employer shall investigate each incident which resulted in, or could reasonably have resulted in a catastrophic release of highly hazardous chemical in the workplace. a) AMREP: The employer failed to investigate and/or develop an incident report of a fire/explosion occurring at the facility that resulted in an catastrophic occurrence. Employees were engaged in forklift operations, gassing operations and other activities involving the manufacture of a variety of aerosols products in which materials such as, but not limited to, acetone, heptane, toluene, propane, A-108 and other propellant gases were stored, handled and utilized. On or about May 23, 2014 a massive fire/explosion occurred that completely engulfed the chemical storage building and other adjacent areas of the facility.
