29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Wingle Construction, Inc. had an employee exposed to struck-by hazards on the head without head protection, while working under an employee on a scaffold, on a commercial construction site in Tuscola, Illinois.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  Wingle Construction, Inc. had an employee exposed to 9 ft., 2 in. fall hazards on a mobile scaffold, due to failure to lock wheels to prevent scaffold movement, on a commercial construction site in Tuscola, Illinois.
