29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Cosgrove Commons, 815-817 Wethersfield Avenue, Hartford, CT. Job Site - On or about 5-30-14 - Two employees were observed installing wood trim on the second fall porch exposed to a 13 foot fall hazards with no fall protection system in use and inadequate fall protection systems being used, respectively.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) a) Cosgrove Commons, 815-817 Wethersfield Avenue, Hartford, CT. Job Site - On or about 5-30-14 - Two employees were observed exposed to 13 foot fall hazards were not using fall protection systems correctly. The anchorage point was not properly secured and the length of the lanyard was improper for one of the exposed employees, while the second employee was not protected by either a personal fall arrest system or a guard rail system.
