29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts (a) Tumbler Area - The employer failed to ensure that adaquate machine guarding was provided to protect employees from struck by and/or entanglement hazards. The entrance to the enclosure (guard rails) leading to the area where two tumblers were located was not equipped with an interlock system. In the alternative: The employer did not ensure that lockout/tagout devices were applied to prevent employees to enter the danger zone of the equipment to perform cleaning and sweeping in the area during operation.
