29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Shop, North Wall:  Employees were exposed to being struck by flying debris created by the large silver pedestal grinder on the right side facing the bench grinder, which had the work rest adjusted at more than one eighth inch and was not capable of being adjusted.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Shop North Wall:  Employees were exposed to being struck by flying debris created by the large silver pedestal grinder which had the tongue guard adjusted at greater than one fourth inch on the right side facing the grinder as the tongue guard was not capable of being adjusted.  (b) Shop, Northwest Corner: Employees were exposed to being struck by flying debris created by the Craftsman pedestal grinder which had no tongue guards on the right and left wheels.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a) Shop, Northwest Corner:  Employees were exposed to electrical hazards as the Milwaukee Heavy Duty Sander did not have adequate strain relief, as the electrical cord where it attaches to the sander, the outer insulation had pulled back exposing interior wiring.
