29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Reese Construction, LLC on or about May 30, 2014 at the address of 222 N. Wood St. Fremont, OH: The employer did not ensure employees were protected from falls while installing new roofing to a residential structure.  Employees performing work in this condition were exposed to falls up to 17 ft. to the ground below.

Wood St. Fremont, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations from elevated surfaces.
