29 CFR 1926.1404(d)(1)(ii):  Before commencing assembly/disassembly operations, the A/D director did not ensure that the crew members understood the hazards associated with their tasks.    a)  Worksite, 101 Mineola Blvd. Mineola NY - Outriggers for a Leibherr 1220 LTM 5.2 Mobile Crane were repositioned from the full to the half position, and set on an area of ground that was not sufficient to support the weight of the crane; on or about 12/18/13.      Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1404(h)(2):  The size, amount, condition and method of stacking the blocking was not sufficient to sustain the loads and maintain stability.      a)  Worksite, 101 Mineola Blvd. Mineola NY 11501 - Outriggers resting on 5'x7'x1 1/2" steel plates were relocated and placed on 6"x6"x3' wood timbers; on or about 12/18/13.
