Main St. and Oak St., Old Forge, PA 18518: An employee was working inside a 7-foot deep excavation next to the Daewoo Hydraulic Excavator without any head protection. Employee was exposed to head injuries from impact on or about May 30, 2014. Abatement certification required for this violation.
