29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about May 28th, 2014, employees in the production areas of the shop were exposed to the hazard of becoming entangled with rotating parts in that the chucks on horizontal metal lathes were unguarded. This hazard was noted to exist on a number of horizontal metal lathes including, but not necessarily limited to, the American lathe, the Sharp-2680B lathe, and the Clausing-Colchester KM-350HD lathe. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(2): Guards on machines were not affixed to the machine or secured elsewhere when attachment to the machines was not possible: (a) On or about May 28th, 2014, employees operating or in near proximity to the Komatsu KM-350HD and FMD band saws were exposed to possible finger amputation injuries in that the non-working portions of the band saw blades were missing their sliding guards. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) On or about May 28th, 2014, employees in the shop area were exposed to possible electrical shock hazards in that an energized extension cord's three outlet receptacle (female) end had separated completely from the cord's outer jacket leaving the receptacle end connected by only its internal conductors. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
