(b) N10189 370th Street Boyceville, WI; The competent person on-site did not adequately and regularly inspect the site to ensure equipment such as drench showers were properly placed, in that the shower station was placed directly near the anhydrous ammonia high pressure receiver so the employee exposed to anhydrous ammonia was not able to access the shower as the high pressure receiver was relieving.
