29 CFR 1910.178(l)(3)(i)(A): Powered industrial truck operators did not receive training on operating instructions, warnings, and precautions for the types of truck the operator was authorized to operate: Employees performing framing tasks at 150 N. Mosley St., Wichita, Kansas were exposed to hazards associated with safe fork truck operations. The operator of the Skytrack All Terrain fork truck was not at or near the controls while personnel were elevated on the scaffold platform.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Employees performing framing work at 150 N. Mosley St., Wichita, Kansas were exposed fall hazards of 15 feet 6 inches to the concrete below. Employees working in the open front scaffold platform elevated by the Skytrack fork truck were not protected from falls in that the personal fall arrest systems were not connected anchorages.
