29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: 830 Boston Turnpike, Shrewsbury, MA: As of May 15, 2020, the employer had not selected the appropriate hand protection to protect to employees operating the Fenn Swaging Machine number 6 from the potential hazards identified by the manufacturer and experienced by employees. The first warning in the manufacturer's manual was to never wear gloves or use rags. On May 15, 2020, an employee inserting material into the Swaging machine received severe hand injuries when the material being inserted violently spun and the gloves were caught on the material being worked.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 830 Boston Turnpike, Shrewsbury, MA: As of May 19, 2020, the employer had not performed the annual periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task and therefore did not inspect whether the procedures were being followed. An authorized employee management official went machine-to-machine to have the company's only other authorized employee explain the procedure for each machine.
