29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his environment to control or eliminate any hazards or other exposure to illness or injury:     On or about May 30, 2014, and at times prior thereto, employees using a freight elevator for construction activities while renovating the building located at 141 N. Rock Island, Wichita, Kansas, were exposed to struck-by hazards.  Employees were not provided adequate instruction on the use of the freight elevator to include safety precautions to address the hazards, and the limitations of the elevator to be used for freight only.     29 CFR 1903.19(d)(1):  Requires certification and documentation that the abatement of the above violation is complete.
