29 CFR 1910.23(e)(1): The stairway railing did not consist of a standard railing, which is a top rail, intermediate rail, and posts, and did not have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level: In the grain cleaning building. On or about May 30, 2014, and at times prior thereto, employees were exposed to a fall of greater than six feet, as the stairway railing did not have an intermediate rail, as they travelled the stairway to access the cleaner on the second floor.

29 CFR 1910.215(a)(4): The work rest of the offhand bench grinder was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest: In the maintenance shop, south side. Dayton 6-inch bench grinder, Model 4Z123, Serial No. 23101506. On or about May 30, 2014, and at times prior thereto, employees were exposed to a struck-by hazard from an abrasive wheel shattering, as the work rest on the bench grinder measured greater than 1/8 inch from the wheel, as they worked sharpening tools such as lawnmower blades and machetes.
