29 CFR 1926.25(a): Debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures during the course of construction, alteration, or repairs:  a) 71 Wintergreen Avenue West, Building 16, Edison, NJ 08820. Employees were allowed to work at the base of the building, the ladders and passageways when they were covered with building materials containing exposed nails exposing them to puncture wound, slip and trip hazards.   Condition occurred on about 05/30/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 71 Wintergreen Avenue West, Building 16, Edison, NJ 08820. The employer allowed his employees to work on and from the roof of a two story Townhouse aka Building 16 doing rip and tear work as well as new installation of roofing materials with no fall protection systems in place exposing employees to fall hazards.     Incident occurred on or about 05/30/14.

29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  a) 71 Wintergreen Avenue West, Building 16, Edison, NJ 08820. Employees were allowed to use a Werner, 20 foot aluminum extension ladder that was damaged when that ladder had not been inspected by a competent person.       Condition occurred on or about 05/30/14.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components were not removed from the jobsite:    a) 71 Wintergreen Avenue West, Building 16, Edison, NJ. Employees were allowed to use a Werner, 20 foot aluminum extension ladder that was damaged in that it had damaged rungs, missing both safety shoes and the right side lock assembly exposing them to fall hazards.      Condition occurred on or about 05/30/14.

Employees were allowed to carry materials, i.e. rolls of felt paper up the ladder when only maintaining two points of contact exposing them to fall hazards.     Condition occurred on or about 05/30/14.
