29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. At the site, the employer did not develop specific procedures for the control of potentially hazardous energy when employees performed maintenance and repair on the GS Manufacturing Over/Under Meter-Mix Dispensing Unit. On or about 2/26/14 an employee sustained a finger amputation while servicing the machine.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. a. At the site, the employer did not ensure that the specific lockout/tagout procedures developed included the removal of lock out devices when performing lockout procedures on the GS Manufacturing Over/under Meter Mix Dispensing Unit.
