29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) The employer did not ensure that employees working with the RT40 Tractor / Ditch Witch (Product Identification Number : CMWH 313XC70000730) on or about 05/29/2014 were properly trained on the operations of the machine. The employees were not provided the training that is enclosed in the operator's manual of the Ditch Witch that specifies limitations and danger zones of the machine causing one of the employees to stand on the back of the machine and having his leg injured by the machine's blades.
