The employees were wearing their harnesses around their waists like body belts and the attachment point was not located at the center of their back.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. (a) Jobsite, 2410 Broad Avenue, Altoona, PA: On or about 05/29/14 the employer had not retrained employees when inadequacies in the employees' knowledge or use of fall protections systems indicated the employees had not retained the requisite understanding or skill necessary to prevent injuries or falls. Employees were not wearing their harnesses properly indicating that retraining was needed to ensure employees understood the requirements of fall protection systems.
