29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 20 May 2014, at the above addressed job site, two employees were standing on the edge of the roof on the building under construction, without the use of fall protection, exposing the workers to a fall hazard of up to approximately 27 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 20 May 2014, at the above addressed jobsite, employees were utilizing a aluminum extension ladder to access the roof of the building under construction without extending the ladder at least 3 feet above the upper landing surface, exposing the employee to a fall hazard of approximately 19 feet.
