29 CFR 1926.452(k)(4): Ladders used to support ladder jacks were not placed, fastened, or equipped with devices to prevent slipping:    Worksite: When the employer used ladders to support ladder jacks the ladders, and the feet of the ladders, were not placed, fastened, or equipped with devices to prevent slipping.  Wood supports and other objects were used in lieu of stable footing to secure the ladders in place.

29 CFR 1926.1053(b)(6): Ladders were not used only on stable and level surfaces unless secured to prevent accidental displacement:    Worksite: When setting up a portable ladder to access a scaffold, the feet of the ladder were place on an uneven surface and the ladder was not secured to prevent displacement.
