29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     On or about May 29, 2014 at a job site located at 593 East Aurora Road, Macedonia, OH, during construction activities including but not limited to masonry installation, the employer did not establish and implement a written respiratory protection program. Employees were required to wear filtering face-piece respirators during cutting and mixing mortar while installing concrete block.

Employees were required to wear respirators during mixing and cutting operations.
