29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: (a) Packaging area, on or about and at times preceding to May 29th, 2014 8-foot platform guardrail systems were missing a section of midrail near the tanks creating a 10-inch gap.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (a) Production Area, on or about that times preceding to May 29th, 2014, employees were not advised as to the voluntary use of 3M 8247 R 95 1/2 mask respirators designated for odor control.
