On or about 5/30/14.    b) Main warehouse, fitting aisle - A thirteen step rolling platform ladder that was approximately 130 inches high had a diagonal cross brace duct taped to its frame. 29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Yard and warehouse - Employees who operated the Daewoo propane forklift (Model #GC30E-3, serial number CV-0087) were not trained per the requirements of the regulation. On or about 5/13/14.    Abatement Note: Forklift operator training consists of a combination of formal instruction (e.g., lecture, discussion, interactive computer based training, video tape, written material, etc.), practical training (demonstration performed by the trainer and practical exercises performed by the trainee), and an evaluation of the operators performance in the workplace.
