29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Note: If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly: a) A Traverse T844 forklift was used to hoist and transport materials (HVAC chases) suspended from the forks by the use of wire rope slings. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.251(c)(16): Wire rope slings did not have permanently affixed, legible identification markings stating size, rated capacity for the type(s) of hitch(es) used and the angle upon it is based, and the number of legs if more than one: a) Wire rope slings provided for employee use did not have permanently affixed and legible identification markings identifying rated load capacities (i.e., working load limits) prescribed by the manufacturer. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
