29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  On or about 16 April, 2014, at the above addressed jobsite, an employee climbed over the top rail of the aerial lift and onto a scaffold platform, exposing the employee to a fall hazard of approximately 37 feet.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about 16 April, 2014, at the above addressed jobsite, an employee working from an aerial did not utilize fall protection, to prevent him from falling out or being ejected from the basket, exposing the employee to a fall hazard of approximately 37 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 16 April 2014, at the above addressed jobsite,  two employees were working from a building edge and another employee was working on a walking platform  without the use of fall protection, exposing the two employees to a fall hazard of up to approximately 24 feet and the other employee a fall hazard of approximately 37 feet.
