29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On June 2, 2014 an employee was not protected from a fall while walking/ working from a commercial canopy, exposing the employee to a fall of 13' 4". Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about June 2, 2014 an employee was not provided with appendix D when a respirator was provided while performing tuck pointing work. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
