29 CFR 1910.215(a)(4): On offhand grinding machines, a work rest was not available to be used to support the work and the work rest available was not adjusted closely to the wheel with a maximum opening of one-eighth inch. Location: grinder On or about June 2, 2014, one abrasive wheel on the grinder did not have a work rest, the second wheel had a work rest that was adjusted to 0.25."
